Agnesian eHealth is a personal on-line view into the electronic health record that Agnesian HealthCare providers use to manage and document care. It is a free web-based tool that allows patients to access their records anytime and anywhere they have an Internet connection.

“This web-based tool is truly a timesaving resource that empowers our patients to take control of their health,” according to Janet Osterholm, system director of Agnesian HealthCare Health Information Management. “As a result, patients can become more proactive members of their care team.”

Here are some of the benefits of using Agnesian eHealth. Patients can:

  • View personal health information (including medications, allergies, immunizations, surgeries and procedures, and a list of health issues)
  • View laboratory results
  • Request appointments
  • Request a medication refill from healthcare providers
  • Manage their personal health profile

Agnesian eHealth is a confidential site that provides individuals with the ability to privately view, store and share health information including medications, allergies, immunizations and select laboratory results.


Agnesian HealthCare patients interested in using Agnesian eHealth can pick up a registration form from any hospital Admitting department or Agnesian HealthCare information desk. Return them to the same location to receive a unique access link. Photo identification will be requested at the time of form submission for verification purposes. Registration forms can also be given to any receptionist, nurse or provider.
